Likeness Release Form

1. YOUR AGREEMENT

By agreeing to this release form, you agree to be bound by, and to comply with, these Terms and Conditions. If you do not agree to these Terms and Conditions, please do not use tick the box.
I, the undersigned, hereby grant permission to Healthitude LLC, its representatives, and employees, the right to use my likeness in photographs, video recordings, or any other media, for the purpose of promotional materials, marketing campaigns, social media, and/or website content.
I understand and agree that these materials may be used in various publications, public relations efforts, or for other related endeavors. I also understand that my name and identity may be revealed in descriptive text or commentary in connection with the images or videos.
I hereby waive any right to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection with them or the use to which they may be applied.
I release, discharge, and agree to hold harmless Healthitude LLC from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking, processing, or reproduction of the finished product.
I understand that Healthitude LLC will make reasonable efforts to use my likeness in accordance with standards of good taste, and I agree to indemnify and hold Healthitude LLC harmless for any failure on my part to meet those standards.
I am of full age and have the right to contract in my own name. I have read this release and fully understand the contents, meaning, and impact of this release.


PLEASE NOTE: We reserve the right, at our sole discretion, to change, modify or otherwise alter these Terms and Conditions at any time. Unless otherwise indicated, amendments will become effective immediately. Please review these Terms and Conditions periodically.
